of the agreement of the parties providing for access to the Addi- tional Facility;

contain information concerning the issues in dispute and an indication of the amount involved,if any;and
e)
state,ifthe requesting party is a juridical person,that it has taken all necessary internal actions to authorize the request.
(2) The request may in addition set forth any provisions agreed by the parties regarding the number ofarbitrators and the method oftheir appointment,as well as any other provisions agreed concerning the set- tlement of the dispute.
(3) The request shall be accompanied by five additional signed copies and by the fee prescribed pursuant to Regulation 16 of the Administrative and Financial Regulation of the Centre.
